HIV and Driver Medicals

People applying for Large Goods Vehicles (LGV) or Passenger Carrying Vehicles (PCV) may need a Medical Report completed by a Doctor. The Form is referred to as a D4 and we keep a stock of these for patients who have their medicals completed here.

Bus and Lorry Drivers

Bus and Lorry Drivers require a Medical Report completed by a registered doctor when:

  • You first apply for an LGV/PCV licence.
  • When you renew your licence on or after the age of 45. If this is the case you need to repeat your medical examination every 5 years.
  • From the age of 65 licences are issued for one year at a time and you will need a medical report every year.
To book a D4 examination just call us on 020 7637 1600 and ask for an HGV Medical Examination appointment.

We will need you to bring a form of photo ID such as Passport or Photo Driving Licence. Please bring a list of any medications with you. Please also bring your regular NHS GP’s full details. Please note you will need to arrange an eye test separately.

We will ask you to pass a urine sample at the clinic.

The cost of the examination and the issue of the Medical Report is £100.

We will take your payment at the time of booking the appointment so please have a valid Debit/Credit card handy. We are happy to cancel and rearrange appointments if needed but we do require 24 hours notice of cancellation otherwise the full cost of the medical will be charged.
